  2. Submit a valid State of CT School Health Assessment Record Form to the Athletic Director’s Office or electronically via Final Forms.

    • Page 2-Completed and signed by medical provider.

    • ***YOUR PHYSICAL EXAM FOR ATHLETICS EXPIRES AFTER 13 MONTHS. AN EXPIRED PHYSICAL WILL RESULT IN INELIGIBILITY***
